In order to successfully change your name, you need to provide the proof of your name change in the form of a marriage certificate, divorce decree, court order, or certificate of naturalization that shows your new name.

If you need an ID card with your new name on it for a bank account or other application, you can ask for a temporary print-out that the agent can give you immediately.
If you forget to do this when you originally change your name, you will have to fill out the SS-5 Form again and bring it to a Social Security office. If you have your online account set up, you can expedite this process by ordering a replacement card online.

According to the new law passed in 2013, Social Security benefits must be received electronically. When you initially set up your Social Security benefits, they will require you to provide a bank account to write funds into monthly. If you get a new bank account, or lose access to your existing bank account, it is essential to update this information if you want to receive your payments.
The fastest way is to call the Social Security office, and they should be able to handle it for you if you able to prove your identity. If not, you will need to go into a Social Security office with the documents to prove your identity and citizenship. At that point they will take the Direct Deposit form and update your bank account.
You can also do this remotely through an online Social Security account. Updating your direct deposit is fast and easy online, but setting up the account is it’s own step.
